Please work with your building Tech Coach to complete the online New Technology Request Form. All forms are sent to your building administrator for approval, then submitted to the Director of Curriculum or Director of Special Services and the District Instructional Technology Coordinator, who will forward requests to IT as needed. Prior to completing this online request form, making a copy of the Technology Request Template Form can be used as a working document to complete all questions before entering information into the Jotform. Before completing the process be sure to check the signed DPA list and CatchOn SPS List.
A Data Privacy Agreement (DPA) is a legal contract between a school or educational institution and a third-party vendor that outlines how student data will be collected, used, stored, and protected. DPAs are designed to ensure that the company complies with data protection and privacy regulations, safeguarding student information. All staff must utilize 3-3-3 Protocol unless rostered by SPS. This is particularly important for students under the age of 13 as when a teacher or the district rosters a student (creates student accounts), we are acting as the agent for parental consent. The 3-3-3 Protocol allows us to set up student accounts without using any personal identifying information from the student.
The login name is a combination of a portion of their first name, last name, and student ID number. It consists of:

Their password is their complete student number.
